In the past I've had instances of the following problem. During backup
operations to a tape, I'd get an error such as this:

"An inconsistency was encountered on the media in miniQIC. Do not
append to this media."

I finally solved this problem, and wanted to pass the solution on to
others.

If I retension the tape this problem goes away. In NT Backup (that
comes with Windows) you can do this from the Tools/Media menu when in
restore mode. I recommend retensioning at least three times, and then
doing a backup to the tape.

There may be other causes for that error, but my solution has worked
consistently for me.
